The Amplify Program is a quarterly rebate program beginning April 1, 2024.

The Amplify Program enables qualifying practices to earn rebates by purchasing ILUVIEN® or YUTIQ®

The Amplify Program rebate is calculated and administered on a quarterly basis with practices accumulating purchase credits toward rebates throughout each calendar quarter. After calendar quarter end, the number of purchased units is compared to rebate program thresholds to determine rebates earned through qualifying purchases.

The Amplify Program restarts every calendar quarter.

Free-of-charge product, replacement product, and compassionate-use product do not count toward rebate thresholds or qualify for rebates.

There are two separate and distinct rebate calculations for ILUVIEN and YUTIQ.
Rebate thresholds
To qualify for rebates, each product has two rebate thresholds, Visionary and Visionary Plus. A bonus rebate for each product may be earned by meeting qualifying purchase thresholds for both products. (See thresholds below and Figure 1: Potential rebate outcomes across the Amplify Program)
ILUVIEN
Multi-Physician Practice:
The Visionary threshold is achieved by purchasing at least 1.5 units of ILUVIEN per quarter per eligible treating physician within the multi-physician practice.
Single-Physician Practice:
The Visionary threshold is achieved by purchasing at least 2 units
of ILUVIEN per quarter for single-physician practices.

BONUS PROGRAM
The Visionary Bonus threshold is achieved if the Visionary purchase thresholds noted above are attained for both ILUVIEN and YUTIQ products. Purchases achieving this Visionary Bonus threshold result in an additional 0.5% rebate on ILUVIEN and 0.5% rebate on YUTIQ purchases made in the qualifying quarter.
BONUS PROGRAM
The Visionary Plus Bonus threshold is achieved if the Visionary Plus purchase thresholds noted above are attained for both ILUVIEN and YUTIQ products. Purchases achieving this Visionary Plus Bonus threshold result in an additional 0.5% rebate on ILUVIEN (total 1% additional) and 0.5% rebate on YUTIQ (total 1% additional) purchases made in the qualifying quarter.

Separate rebate payments will be made for each of the ILUVIEN and YUTIQ rebate programs.
Rebate payments are made approximately 60 days after qualifying quarter end.

Qualifying information
For a multi-physician practice to qualify for a rebate, purchases by the practice must exceed the threshold multiplied by the number of treating physicians.
Example: For Visionary, a 4-physician practice qualifies with a minimum of 6 units of YUTIQ ordered in a calendar quarter. For Visionary Plus, a 4-physician practice qualifies with a minimum of 12 units of YUTIQ ordered in a calendar quarter. The same example would apply to the ILUVIEN program.
A practice is defined as an owner who is the responsible financial party. Multiple locations may fall under a sole responsible financial party.
Eligible physicians are practicing ophthalmologists who specialize in treating retina disease. Practices must identify eligible physicians at the start of each qualifying quarter.
Additional information
Alimera reserves the right to exclude any practice at any time for a variety of reasons.
Government pricing programs and teaching hospitals are ineligible to participate in the Amplify Program.
Practices may have independent government or claims-based reporting obligations with respect to rebates received. Alimera will provide reporting of rebates paid on the rebate statement.
Practices participating in the Amplify Program represent that they are legally eligible to receive the rebates possible to be earned through the program and will fully and accurately report the discount on applicable claims. Practices may request, and Alimera Sciences will provide, purchase threshold reports or other information required under 42 C.F.R. 101.952(h) at the close of each calendar quarter.

What does our practice need to do to get started in the Amplify program?You are eligible to receive a rebate just by purchasing ILUVIEN or YUTIQ. You can proceed further to set up your account so that you can receive payments when you purchase the required amount of product, or you can opt-out if you decide that you do not want to participate in the rebate program.

What information does the practice need to provide to receive payment for a qualifying rebate?You will be asked to provide: Practice name and contact information Practice Manager contact information W9 for the practice List of Ophthalmologists specializing in retina treatments and their NPI numbers Bank Account information if choosing to have electronic deposits made for rebate payment (alternatively, you can elect to have Checks cut and mailed to your practice)

How many units per quarter does it take to qualify for a rebate?See the description of Visionary and Visionary Plus thresholds in the Amplify Overview and Amplify Details materials or here

Do we need to qualify with both products to obtain a rebate?Each product has its own rebate program, so you can qualify by purchasing either ILUVIEN or YUTIQ. If you qualify for rebates with both products, you may be eligible for a bonus rebate on each product (see details on Bonus program here).

Can one product contribute to the rebate calculation for another product?No, the rebate calculation is by individual product.

When does Day 1 begin for calculating units toward my rebate?The Amplify rebate program applies to all purchases of ILUVIEN or YUTIQ made on or after April 1, 2024. Purchases made within each calendar quarter will apply toward the rebate thresholds for that quarter.

What form of payment will be sent for the rebate?The payment will be sent either electronically by wire or ACH, or by check, whichever you select.

When do we receive our payment?Approximately 60 days following the qualifying quarter.

Is the unit calculation rounded up?No.

Are unit samples or free of charge units included toward calculating the rebate?Free of charge units, study units or replacements do not count toward the rebate threshold.

What data is being used to calculate the rebate?Calculations are based on the sales data received from your distributor, either Besse or McKesson, for the applicable rebate period.

What if our rebate calculation is different from Alimera’s?You should email the Amplify team at amplify@alimerasciences.com and we will follow up to address discrepancies.

Will consignment units be used in the calculation?Consignment units will count if used and followed by a replacement being ordered.

What if a doctor is working for two different practices? Which practice will receive credit for purchasing the product?Credit for the unit will apply to the practice that purchased the unit. Rebate payments are made to the financially responsible party, not the physician.

Do Fellows count toward the overall number of treating physicians used to calculate the threshold?While a Fellow is a physician and can treat patients, they are under the supervision of the attending physician. Therefore, Fellows do not count toward the total number of retina treating physicians in the practice but credit for units purchased for treatment by a Fellow will be given to the practice.

Are practices in all states eligible for participating in the Amplify program?Yes, practices in all states are eligible to for the Amplify rebate program. However, physicians and practices are responsible for complying with any local restrictions or regulations.

If a practice has 'X' number of doctors of all specialties, but only 'Y' retina doctors, how is the threshold determined for the practice?The 'Y' number of retina doctors will be used to calculate the threshold.
